Work on your survey2.html document to get it into a final, pristine and perfect layout. Add some images to your Submit and Reset buttons, add password textboxes or extended option lists, depending on your need.
Look at this revised survey, fill it out and submit your answers.
Save your document as survey3.html and run it through a browser. Make sure that your submitted information is coming to your email correctly.
